Bureau of Immunization PublicationsThe Bureau of Immunization produces a variety of materials that are available for use by County Health Departments, private providers, and immunization partners. Documents with stock numbers may be ordered directly from the Department of Health Distribution Center by sending form DH 1464, 7/06, along with a shipping fee. Print out the order form and follow the instructions on the form.

Many Bureau of Immunization brochures are designed for affiliate customization of the rear panel to display the affiliate logo, company name, address, e-mail, web address, and phone number, or a brief local immunization message. The bureau grants immunization partners limited rights to display an affiliate logo, provided that no part of the Bureau's or Department of Health's material, logos or brand are altered in any fashion. Additionally, the bureau's products may not be sold. Images used in these publications are copyrighted and were used with permission of the copyright holder and are not in the public domain. The Bureau of Immunization or Florida Department of Health has licensed these images for use in these materials in the form presented on this website and they may be used without seeking further permission.
