Vaccine Information Statements

Prior to the administration of each dose of vaccine covered under the National Childhood Vaccine Injury Act (NCVIA), all public and non-public providers are required to provide the client, parent, or guardian with a copy of the most current Vaccine Information Statement (VIS), prepared by the Centers for Disease Control and Prevention (CDC). They explain to vaccine recipients, their parents, or their legal representatives both the benefits and risks of a vaccine. They are available in English, Spanish, and other languages.

Additional VISs are available for some vaccines that are not covered under the NCVIA. These VISs must also be provided to the client, parent, or guardian for all vaccines purchased.

Clients or their legal representatives will be given the opportunity to read this information or have it explained to them and will have a chance to ask questions concerning the information, which will be answered to their satisfaction.
