Information for the CommunityVaccine Information StatementsPrior to the administration of each dose of vaccine covered under the National Childhood Vaccine Injury Act (NCVIA), all providers are required to provide the client, parent, or guardian with a copy of the current Vaccine Information Statement (VIS), prepared by the Centers for Disease Control and Prevention (CDC). VIS explains both the benefits and risks of a vaccine to vaccine recipients, their parents, or their legal guardian. They are available in English, Spanish, and other languages, from the Immunization Action Coalition. Additional VISs are available for some vaccines that are not covered under the NCVIA. These VISs must also be provided to the recipient, parent, or legal guardian for all vaccines administered. Recipients or their legal guardian will be given the opportunity to read this information or have it explained to them and will have a chance to ask questions concerning the information, which should be answered to their satisfaction prior to receiving the vaccine.
